On 29th July 2021, Phnom Penh Capital Administration issued the notification to inform the public regarding the COVID-19 vaccination for children and adolescents between the age of 12 to below 18 in Phnom Penh Geography.
The first dose of COVID-19 vaccination for children and adolescents between the age of 12 to below 18 in Phnom Penh start from 1st till 14th August 2021 and the second dose will start from 22nd August till 4th September 2021.
The children and adolescents between the age of 12 to below 18 that go to get the vaccine need to bring the following documents:
- Children age between 12 to below 15 need to bring birth certificate, or resident book, or family book, or passport
- Adolescents age between 15 to below 18 need to bring birth certificate or Cambodian ID card, or resident book, or family book, or passport
The vaccination for children and adolescents between the age of 12 to below 18 must have the written consent for vaccination from:
- Parents / Guardians. In the event that the child or adolescent stays away from the parent or guardian, the parent / guardian is required to complete a written consent form and send it online to their child or relative to provide at the vaccination site.
- Managers for orphans in the center
- For the homeless child or adolescent, there need to be clarification letter from the competent authority
